CANCELLATION POLICY

We understand that sometimes things happen that cause a conflict with your scheduled appointments.
However, not showing up or canceling at the last minute, leaves us unable to fill a spot that other clients
are waiting for. Please respectfully provide a minimum of 24 hours’ notice to cancel or reschedule your
massage therapy appointment. Thank you for understanding. By signing this form, I understand and agree
that not showing up for my scheduled appointment or canceling with less than 24 hours notice, will result
in being charged a cancellation fee of $50 for a 60 minute appointment or $75 for a 90 minute
appointment AND future appointments will require a non-refundable, full payment in advance.
